Use the public share link from your form service, not the admin view. On Google Forms, this is the "Send" link, which usually starts with https://forms.gle/ or https://docs.google.com/forms/.../viewform. On Typeform, it is the share link ending with your form's slug. On Microsoft Forms, it is the "Collect responses" link.
Open the link in a private browser window first to confirm it works without a login. If it asks for a sign-in, scanners will not be able to open it either.
